Letter of Agreement/Contract for Trunk Show/Workshop
This is a contract between ________________________________________________ and Priscilla Creations for a trunk show and or a workshop. Date ________________ Time______________ and type of presentation trunk show workshop_________________. Circle the appropriate one.
The fee for the services is _______________________. ______________agrees to pay Priscilla Stultz the above mentioned fee for the appearance described above. It shall be the responsibility of Ms. Stultz to present any necessary documentation; receipts or statements for services and expenses before reimbursement of fees are paid. _____________________also agrees to the following:
*Transportation: Ground transportation shall be paid the current US Government rate for mileage. __________miles x ________ = _________.
Airline ticket if presentation is more than 200 miles away. Pick up and delivery to the airport.
Meals (if deemed necessary) before or after the presentation. Breakfast or Dinner if the presentation is more than 100 miles away.
Lodging will be provided in a hotel, if the travel is more than 100 miles. Depending on the time of the presentation, the lodging provided may be the evening before or after the presentation.
In the case of illness, accident, acts of God or unavoidable circumstances, Ms Stultz fails to appear or ________________________ is unable to hold the event scheduled and a mutually satisfactory date for the appearance can not be arranged it is hereby agreed that this contract is null and void and that there shall be no claim for damages by either party, except that __________________________shall be responsible for the cost of the airline ticket, if it has been purchased.
